If your child has a MyCooper account (Cooper medical record) but you do not want proxy* access.
You will need to complete registration/demographic information at the time of the appointment. To reduce the time in line at our location, please enter accurate information. If you do not have separate home and mobile phone numbers, please enter your primary number in both fields.
Please bring your child’s insurance card with you to your vaccine appointment. Your insurance may be charged, but you will not receive a bill for any co-pays. If you do not have insurance, the vaccine will be provided at no cost to you.
* Proxy access allows you to view your child’s vaccine record as well as the rest of their medical record.

If you have been seen by the selected provider in the past three years.
Your appointment is accident-related and would qualify for workers comp or auto insurance.
You need an appointment at the Advanced Care Center or Ripa Center.
You would like to reschedule your appointment.
Cooper University Healthcare accepts insurance from a variety of health plans. Please check with your health plan to confirm that our services are
included. Our providers and hospitals may participate in some, but not all, products offered by a health plan. Coverage limitations are dependent
on individual group contract. For questions about insurance coverage, place contact your insurance carrier directly. Regardless of coverage level,
it is best to bring a form of payment in the event that a copayment is required.
If you are not a New Jersey resident and:
You are covered by Blue Cross Blue Shield, do not select the HORIZON BCBS option. Please use the BLUE CROSS BLUE SHIELD option.
You are covered by Medicaid, please use the OUT OF STATE MCD option.
